The purpose of these videos is to give you and your family information about services here in Rhode Island that can help you or your loved one live independently and safely at home and in the community. We want you to know that you have many good choices. Our goal is to help people live where they want to live, which for most people is at home.

The videos were paid for with a federal government grant to the State of Rhode Island to help elders and adults with disabilities live in their homes and in their communities.

The first 2 videos will discuss:

• How you and your health care professionals can determine what kind of help you or your loved one may need to live independently at home.
• What you can do now to plan ahead for your return home.
• Services that are available to help, how to find those services, and how the state may be able to help support the costs.
• Some other useful sources of information.

Discharge planners play a critical role in helping elders and adults with disabilities live safely at home after being in the hospital or a nursing home.  The purpose of this video is to give a thorough understanding of the options available to help Rhode Island’s elders and adults with disabilities return home. It is also intended to give discharge planners information about the process of assessing needs, the different players involved and their roles, and the services that are funded by the state of Rhode Island.  While this video was created for discharge planners throughout Rhode Island, it can also be helpful to anyone with an interest in this subject. This video is part of a larger effort by the state of Rhode Island to improve our infrastructure so that we can help more elders and adults with disabilities live safely and as independently as possible in their homes and their communities.
