The Department of Human Services Long Term Care
Program provides home and community based services and
coverage for the cost of nursing home care.
Individuals over the age of 65 and those with disabilities
must meet certain financial and program guidelines, and
need the level of care provided by a nursing facility.
Eligible individuals may choose to receive services at home
or in a nursing home.
